WHAT WE OFFER

The Geriatric Assessment and Intervention Network (GAIN) Clinic is part of a network of coordinated healthcare services that provide clinical care and education to ensure the best care for frail older adults and their families. Services include:

  • Comprehensive geriatric assessment

  • Assessment of memory and thinking

  • Assessment of balance, mobility and falls risk

  • Medication and supplement review

  • Management of responsive behaviours

  • Caregiver support and navigation

The GAIN clinic also offers interventions to patients, including:

  • Education regarding chronic disease self-management

  • Teaching about dementia and behavioural care

  • Advance care planning

  • Referral to community supports

  • Where appropriate, collaboration with home care to facilitate in-home supports, long-term care application and more

  • De-prescribing and medication adjustment

  • Initiation of treatment for dementia

WHO WE ARE

An interprofessional team of healthcare professionals works collaboratively with patients and families and their primary care providers to strengthen supports and capacity for older adults whose health concerns threaten their ability to live independently at home (including retirement residences). Our GAIN teams includes:

  • Behavioural Support Nurse

  • Geriatricians

  • Healthcare Case Managers

  • Nurse Practitioners

  • Occupational Therapists

  • Personal Support Workers

  • Physiotherapists

  • Pharmacists

  • Social Workers

SEEKING CARE

For patients visiting the GAIN clinic, for the first time, please bring:

  • Your Health Card

  • A family member or someone who knows your healthcare history

  • Walking aids

  • Eyeglasses

  • Hearing aids

  • All medications, including over-the-counter products and supplements (please bring all medication containers, bottles, eye drops, cream etc.)

  • A light lunch or snack, as your initial assessment may take two (2) to three (3) hours

Following the clinic visit, a summary of recommendations will be sent to your referring primary care provider or specialist.

HOW TO REFER TO GAIN

We accept referrals from any source, including nurse practitioners, physicians, and patients and  families directly.
