WHAT WE OFFER

The Complex Continuing Care (CCC) program at Peterborough Regional Health Centre (PRHC) meets the highly specialized needs of adults with complex physical and/or cognitive conditions whose needs cannot be met at home or in the community.

WHO WE ARE

The multidisciplinary healthcare team that patients may see, includes:

  • Nurse Practitioner

  • Occupational Therapy

  • Physician

  • Physiotherapy

  • Recreation Therapy

  • Pharmacy

  • Registered Nurses (RN)

  • Registered Practical Nurses (RPN)

  • Registered Dietitian

  • Social Work

  • Speech Language Pathology

  • Spiritual Care

  • Volunteer Support

Through the CCC program, we work to facilitate an exemplary multidisciplinary team environment where all healthcare professionals, patients and family members are incorporated into the process for making decisions about care.

SEEKING CARE

The CCC program sees:

  • Patients with chronic complex medical conditions

  • Patients requiring low-tolerance, long-duration rehab for varying conditions

  • Patients who require on-going non-acute support until they meet admission criteria for other specialized areas such as rehabilitation or palliative care

The goal of the CCC program is to meet the patient’s physical, emotional and social needs and improve quality of life, with a focus on discharge to the community, with or without supports.

HOW TO REFER TO COMPLEX CONTINUING CARE

Patients who come to CCC may be referred to these beds by a physician or nurse practitioner within the hospital or from the community.
