Health Information

The role of the Health Records department is:

  1. To maintain accurate and complete medical records that can act as a means of communication amongst physicians and other health care professionals contributing to the care of a patient to ensure continuity of patient care and to furnish documentary evidence of the course of every patient's illness and treatment during each hospital visit;
  2. To establish and recognize best practices in the management of privacy, confidentiality and security of health information;
  3. To apply the technologies used to collect, access, store and transmit information in all its forms;
  4. To provide statistics which may aid in determining trends or problems and which may help to achieve better/optimal patient care;
  5. To serve as an information center for use in research and education.

Accessing your Health Records during COVID-19
The Health Records office is temporarily closed to the public. We apologize for any inconvenience. To request record copies, please download print and complete this formRequests will still be processed. They can be emailed to roi@prhc.on.ca or faxed to 705-876-5146.

Once requests are complete, we will contact you to arrange payment via credit card and your request will be mailed to you. Please contact us at 705-743-2121 x. 3296 with any release of information questions. Thank you for your understanding.

You can also use this form to request the Health Information of a deceased, minor, or incapacitated individual, if doing so, please refer to these linked instructions.

Individuals making third party requests (e.g. Insurance company, lawyer etc.) must download, print and complete this form.
