Along with all hospitals in Ontario, PRHC reports our rates of hospital-acquired infections and other important patient safety indicators routinely.

Patient Experience Surveying by Email

Feedback from our patients about their experience receiving care at PRHC is important for our organization to understand what we are doing well and where we need to focus our improvement efforts.

PRHC is now collecting patient feedback digitally by sending some of our patient experience surveys by email using a digital platform called Qualtrics XM. Collecting feedback digitally allows for easier collection, storage, analysis, and comparison of information. Patients who provide their consent for use of their email address will receive a survey if they have an overnight stay in the hospital or received treatment in the Emergency Department.

Frequently Asked Questions

Guidance from the Information and Privacy Commissioner (IPC) of Ontario indicates that it is best practice to obtain express consent to send a patient survey using the patient’s email address. We document that express consent was received in the patient’s electronic health record and only send surveys to the patients who have given their consent to do so. 

If you have given your consent and provided your e-mail address, you will be sent a survey within a week of receiving treatment in our Emergency Department or being admitted to one of our inpatient units.

Sample inpatient question: Did you get the support you needed to help you with anxieties, fears or worries you had during this hospital stay?

Sample Emergency Department question: Did care providers do everything they could do to ease your discomfort or symptoms?

Your survey responses are not added as part of your health record, or visible to members of your healthcare team. Survey responses are stored on a separate database and results are presented to PRHC leadership (e.g. managers and directors).

These results are used to make improvements on the unit and celebrate successes. If a comment requires follow-up with a specific unit or staff member, the comment itself will be shared, but the identity of the patient will not be shared.

Yes, if the patient and the other person providing the email address agree (verbal consent) and understand that a survey about the patient’s experience may be sent to the provided email address. The survey itself, should be completed by the patient.

PRHC is committed to continuously improving patient care. Your shared experience helps us identify areas for improvement. We are also able to compare the results of our patient experience surveys with other hospitals in Ontario.
