REACTIVATION CARE CENTRE

PRHC’s Reactivation Care Centre is a new inpatient care space designed to support patients in continuing their care when they no longer require specialized, acute hospital services. The focus of this unit is to help patients improve their quality of life and achieve their highest level of ability. The Reactivation Care Centre provides dedicated support for identified patients and their families through a time-specified, short-stay admission, helping each patient to achieve specific goals that will allow them to safely transition back into the community.

  • Caregivers: An integral part of the care team, who will be included in all aspects of care and daily activities. Caregivers will be updated regularly and be informed on progress and changes in treatment and conditions.

Personal Items to Bring During Your Stay

Put patient/your name on all items when possible. Please note that laundering your clothing is the responsibility of the patient and/or caregiver. 

  • Bring all current medications, including prescription and non-prescription/over the-counter medications, in their original containers and packaging.

  • If you use eyeglasses, dentures and/or hearing aids, please bring them with you. For your hearing aids, remember to bring along extra batteries.

  • You will be getting dressed each day, so please ensure you have appropriate day clothing, pyjamas or nightgown, bathrobe, slippers and a pair of sturdy shoes with non-slip soles.

  • You will also need a toothbrush, toothpaste, soap, shampoo, brush or comb, body/hand lotion (unscented only), cosmetics and shaving items.

Room Amenities

  • Each room is equipped with a bed, overbed table, bedside cabinet and chair.
  • Electronic whiteboards are displayed for each patient that will be updated at a minimum of once per shift. Patients and families are encouraged to add to the information.

Family presence

We know how important it is for you to have your family members and partners-in-care involved your care and care planning. Our family presence policy expands on the standard visiting hours to 24/7 access for partners-in-care, allowing them to spend more time with you.

A care partner is an individual who is:

  • Identified by you, to be the primary person involved in your care
  • The primary communicator with our healthcare team
  • Someone who will be involved in your care planning and decision-making
  • A person who is considered to be a member of the care team, not a visitor

Discharge Planning

  • The expected length of admission on the unit is 8 to 10 days.

  • The team will start working with you on admission to help develop your discharge plans back to the community.

  • The discharge planning process will be supported by the PRHC at Home Transition Navigators. This team will ensure that patients have a fully-integrated, comprehensive care program provided in the community.
