Ensuring patients receive the most appropriate and timely care is critical to slowing the progression of chronic kidney disease and delaying the need for dialysis. At the Nephrology Clinic, we provide a single point of entry into Trillium Health Partners’ Chronic Kidney Disease Program, which makes it easier and faster for physicians and patients and their families to get the care they need, when they need it. Patients with early chronic kidney disease that is mild to moderate (stages 1, 2 and 3) are followed up in this clinical program.

The Multi-Care Kidney Clinic (MCKC) are specialized, multidisciplinary clinics consisting of a team of diverse yet complimentary healthcare professionals with specialized skills to support patients and their caregivers through shared and person-centered decision-making conversations. MCKC’s are important because they provide care to patients at a critical stage of advanced Chronic Kidney Disease (CKD). During this stage, patients are progressing towards End Stage Renal Disease (ESRD) and need to make several crucial, stressful and often complex decisions about their future course of care.

Glomerulonephritis, also known as GN or glomerular disease, is a group of rare diseases that cause inflammation of the glomeruli, the parts of the kidneys that filter excess fluid from the blood. GN disproportionately affects younger people. With the right treatment time from the right team, their disease can be managed effectively to prevent kidney disease from progressing.

Patients living with GN require specialized, multidisciplinary GN care to prevent or delay progression of disease in accordance with the appropriate GN acuity level. If patients with GN are no longer responding to treatments, referral to MCKC should be considered.  GN care may be delivered in partnership with MCKC to meet patient care needs.
