All children and families who are part of PDEC also have access to a registered social worker for assistance with a range of emotional and mental health concerns related to living with diabetes or concerns that get in the way of managing diabetes.
PDEC APPOINTMENTS
Patients and families of PDEC attend clinic appointments with the pediatrician and PDEC team three times a year. During these appointments, the healthcare team reviews your blood sugar trends, insulin and other medication doses, and advises on insulin or medication adjustments.
The team will answer any questions or concerns you may have about improving your diabetes management – including nutrition, mental or emotional health and any issues you feel are interfering with your day-to-day diabetes management. We also enjoy celebrating your successes!
Here’s what you should bring to your diabetes clinic appointment:
INSULIN PUMP PROGRAM
The Government of Ontario supports people living with Type 1 Diabetes through the Assistive Devices Program to access insulin pumps and supplies. Here at PDEC, the registered nurse and registered dietitian will help you know about all the latest insulin pump options, order insulin pumps, provide education on preparing for and using insulin pumps and will get you started on pump therapy. Regular check-ins are offered to help you best manage your Type 1 Diabetes on an insulin pump.
NUTRITION & FEEDING CLINIC
Children and families have access to specialized nutrition supports through the PDEC. Whether your concerns are new or are long-standing, specialized dietitians will help your family gain knowledge and support families with feeding/nutrition difficulties. Specialized information and support is also available for families interested in advanced nutrition topics to improve diabetes management.
PERSONAL HEALTH IMPROVEMENT TEAM CLINIC
Children and families living with Type 2 Diabetes will be supported by both the Pediatric Diabetes Education Centre and the Personal Health Improvement Team (PHIT). Lifestyle supports include access to a multi-disciplinary team including an exercise specialist. Our registered nurse, registered dietitian and registered social worker work in both clinics to best support children and families in making healthy lifestyle changes.
